Routine for assessing and describing the standard 12 lead ECG:
1. Name/Demographics/Notes on ECG?
2. Conventional calibration? (25mm/s, 10mm/mV)
3. All 12 leads visible?
4. Start with lead II (rhythm strip)
- Narrow vs broad complex? (Normal QRS duration <0.12s or 3 small squares)
- Rate
- Rhythm
- PR Interval (Normal < 0.2s or 5 small squares)
5. Axis
- Need only look only at I, II and aVF
- Normal axis (Lead I +ve and aVF -ve)
- Right axis deviation (Lead I -ve and Lead AVF +ve)
- Left axis deviation (Lead I +ve plus II & AVF -ve )
6. ST Elevation?
7. ST depression?
- Upsloping ST segment 2mm, depression, 80msec after J point
- Horizontal ST segment 1mm depression, 60-80msec after J point
- Downsloping ST segment >1mm from top of curve to PQ junction
8. T-wave inversion?
