The "Pottassium sparing diuretic". Competitive aldosterone receptor antagonist acting in the renal cortical collecting ducts. This results in decreased reabsorption of sodium and water, while decreasing the secretion of potassium. Onset and offset of action is over several days as it relies on steroid pathways and alteration of gene transcription. Used in the management of chronic liver disease, cardiac failure, Conns syndrome. Consider the presence of hyperkalemia.
