Anomalous left coronary artery from the pulmonary artery (ALCAPA) is a congenital heart defect in which the left coronary artery originates from the pulmonary artery instead of from the aorta.
As the patient's pulmonary vascular resistance falls normally in the months following birth, coronary perfusion of the myocardium supplied by the LCA reduces resulting in myocardial ischaemia and ventricular dysfunction.

Patients generally present in the months following birth with signs of cardiac failure.
The delayed presentation is due to the fact that their pulmonary vascular-resistance has begun to fall as is expected about this age. This results in reduced or retrograde flow through the LCA resulting in myocardial ischemia.
