"Dabigatran etexilate is a prodrug that specifically and reversibly inhibits both free & clot-bound thrombin [Direct thrombin inhibitor]. The drug is absorbed from the gastrointestinal tract with a bioavailability of 5%. Once absorbed, it is converted by esterases into its active metabolite, dabigatran. Plasma levels peak at 2 hrs. The half-life is 8 hrs after a single dose and up to 17 hrs after multiple doses." refl.in/dz refl.in/e0

Existing coagulation tests are not validated for use with dabigatran. In an emergency, the most accessible qualitative tests are activated Partial Thromboplastin Time (aPTT). An aPTT >80 seconds is associated with a higher bleeding risk. refl.in/mb

"Although there is no specific antidote to antagonise the anticoagulant effect of dabigatran, due to its short duration of effect drug discontinuation is usually sufficient to reverse any excessive anticoagulant activity. Dabigatran can also be dialysed in patients with renal impairment. In instances of life-threatening bleeding, where conventional measures have failed or are unavailable, other non-specific prohaemostatic agents such as recombinant activated factor VII and prothrombin complex concentrates can be considered." refl.in/jh
