Nasopharyngeal airway (NPA) with connector from an ETT one size smaller than the size of the NPA. Connect this to the anesthetic machine with 6L/min oxygen. (this allows you to ventilate by closing the mouth and other nostril if required). Topicalise the oropharynx with cophenylcaine spray. Propofol and remifentanil infusion to sedate and minimise coughing on stimulation. Proceduralist should topicalise the vocal chords and trachea as the bronchoscope is introduced.
