I know a number of E.D. physicians were v. reticent to use it except in potentially life threatening circumstances due to concerns of causing difficult-to-reverse seizure activity. I assume this is more of a problem among Benzo dependent patients, but wonder if it is also a consideration in Anaesthetics cases.
Precipitating seizures is definitely a consideration, although if it avoids intubation there is a clear benefit that needs to be weighed against the risk of seizures. Titrating to effect and use in the right patient population (eg those without known seizure disorders or intracranial pathology) is probably reasonable.
The average elimination half-life of flumazenil is 53 minutes.
