If a patient is refusing to have the mask placed on their face for preoxygenation, try placing the end of the circuit in their mouth instead. The liqorice stick is probably the most comfortable piece of the circuit to use for this purpose.

The combination of low dose dose fentanyl (2mcg/kg) and esmolol (2mg/kg) is more effective than the same dose of either agent alone in blunting the tachycardic and hypertensive response to laryngoscopy and intubation following rapid sequence induction. It is more effective in blunting the tachycardia but was less effective in blunting the hypertension than fentanyl 5mcg/kg. The combination then provides an alternative to a higher dose of fentanyl for blunting the haemodynamic response to laryngoscopy and intubation during rapid sequence induction in healthy patients. refl.in/je

When administering fentanil to blunt the sympathetic response to laryngoscopy, doses of 3-5mcg/kg will generally be required. Peak effect will be seen at approximately 5mins. Chest wall rigidity may make mask ventilation difficult, so time administration of muscle relaxants to counter this.

If hypotension is encountered on induction, a rapid means of increasing preload is to have somebody (eg theatre technician) hold up the patient's legs at 45 degrees.

Running through an ABCDE after induction is a good habit to adopt to minimise omissions.
A - (note ETT depth, bilateral air entry, and ensure adequately secured).
B - (SpO2 monitor working, correct ventilation settings).
C - (ECG attached if necessary, check NIBP is cycling).
D - (Volatile anesthetic, Muscle relaxants, Antibiotics, Antiemetics).
E - (eyes taped, positioning, warming blanket, fluids, urinary catheter).
