Once the indication for using high flow has resolved, and the patient is stable in 40% oxygen the flow can be weaned to 1-2 L/min with FiO2 of 100% via standard nasal prong therapy, or oxygen therapy ceased. Generally there is no need for a prolonged weaning process, better to be on high flow, standard low flow or off oxygen therapy. " (HFNP, PICU Intranet Guidelines)
Once a patient is no longer requiring HFNP can we please ensure their nasal prongs are not connected to a blender with an FiO2 of less than 100%, Ideally they should be connected to a low flow meter not connected to a blender. Once on standard nasal prong therapy if the patient does not need as much oxygen please wean the flow rate rather than the percentage. ie: 2lt/min to 1.5lt/min (100% Fi02) until weaned off.
