In the chronic alcoholic, or other patients at risk of thiamine deficiency, it was once thought that administration and metabolism of glucose may use up residual stores of thiamine placing the patient at risk of wernicke encephalopathy.
From this review of the literature the suggestion was that if this is the case at all, it may only be so in patients with prolonged glucose infusions and that correction of hypoglycaemia is a priority -
"Mounting case report evidence suggests that prolonged glucose supplementation without the addition of thiamine can be a risk factor for the development of Wernicke encephalopathy. Based on our findings, a delay in giving glucose to hypoglycemic patients cannot be recommended at this time, although prompt thiamine supplementation after or concurrent with a return to normoglycemia is recommended."
