If requiring prolonged hypotension and are using TIVA, avoid increasing the rate of propofol to achieve hypotension as this may result in prolonged anesthesia. Consider adding a short acting agent such as remifentanil to achieve the desired reduction in blood pressure.

Weigh up the risk of end-organ damage from hypotension with the consequences of excessive blood loss. When hypotension is being performed to reduce bleeding in non-life threatening situations, minimise the degree of hypotension (max 10-20% reduction) in patients at elevated risk of end organ damage (elderly, history of IHD/CAD or CVA)
