Consent: Epidural will reduce discomfort of contractions, but they will continue to feel pressure. Use of epidural anesthesia has not been shown to increase the risk of cesarian section, nor poorer outcomes for the neonate. It is a safe technique, however every procedure we perform has risks. 80% work well, 20% may be patchy requiring some intervention, & occasionally it may not work at all.
Common risks are a temporary fall blood pressure & light headedness which can be treated. 1 in 200 chance of PDPH. Increased risk of instrumental delivery (?risk). 1 in 100,000 chance of a serious complication (permanent nerve damage/paralysis, infection).

Low dose epidural in labor does not appear to increase the risk of cesarian section, although it may delay second stage labor and increase the risk of instrumental delivery. There appears to be no evidence of poorer neonatal outcomes. Review article 2004. refl.in/ft

If placing a pre-contraction labour epidural in pregnant cardiac patients (eg HOCM), commencing as an infusion without a loading dose may make it difficult to achieve an adequate block. Consider a loading dose combined with an appropriate vasopressor infusion if concerned about vasodilation and fall in preload. #VM #MMC
