In severe preeclampsia, an initial loading dose of 4g (8 mL of the 50% solution made up to a safe 20% concentration for IV injection by diluting up to a minimum total volume of 17.5ml) should be given. Intravenous doses should be diluted to a concentration of magnesium 20% or less. For intravenous dosing, each 5 mL ampoule of DBL Magnesium Sulfate Concentrated 50% Injection should be diluted by adding at least 7.5 mL of a compatible solution. refl.in/f/mgso4.pdf

Monitoring serum magnesium levels and the patient’s clinical status is essential to avoid the consequences of overdosage. Clinical indications of a safe dosage regimen include the presence of the patellar reflex (knee jerk) and absence of respiratory depression. Serum magnesium levels usually sufficient to control convulsions range from 3 to 6 mg/100 mL (2.5 to 5.0 mEq/L). The strength of the deep tendon reflexes begins to diminish when magnesium levels exceed 4 mEq/L. Reflexes may be absent at 10 mEq/L, where respiratory paralysis is a potential hazard. refl.in/f/mgso4.pdf

Magnesium infusion in the management of asthma. Use 50% Mg Sulphate - 0.1ml/kg (50mg/kg) over 20 mins, then 0.06ml/kg/hr (30mg/kg/hour) by infusion.
Aim to keep serum Mg between 1.5 and 2.5mmol/L.
