If the tip is inadvertently folded on itself at the level of the murphys eye ( the weakest part of the tube) during insertion ( eg pressing on the tracheal wall) it may occlude the ett completely.

When looking at the numbers on the tube to determine depth at chords, it is easy to misread the number describing the tube size (eg 4) with the depth marking for 4 cm and inadvertantly insert the ETT too far.
To see the correct marking the tube needs to be rotated 90degrees counter-clockwise after it has passed through the cords.
