Airway obstruction in this context may actually be due to laryngeal oedema caused by obstructed venous return secondary to the wound haematoma.
Cutting the stitches (although necessary) in this scenario will not necessarily relieve the obstruction after it's occurred, but it can be prevented if the stitches are removed and the haematoma is decompressed before the laryngeal oedema worsens. #ref

Cutting stitches will likely result in significant blood release onto the patient's head and neck. This may make mask ventilation difficult. Have sufficient dressings to manage this.
