A cholinesterase inhibitor used in the treatment of myasthenia gravis and to reverse the effects of muscle relaxants such as gallamine and tubocurarine. Neostigmine, unlike PHYSOSTIGMINE, does not cross the blood-brain barrier.

Reversal of non-depolarising muscle relaxants

Neostigmine 0.05-0.07mg/kg with Atropine 20 mcg/kg

< 25 kg:
• 1.25 mg Neostigmine, 600 mcg Atropine in 3ml syringe (1/2 Adult)
• Dilute to 3 ml
• Give 0.1 ml/kg

> 25 Kg:
• 2.5 mg Neostigmine, 1200 mcg Atropine in 5 ml syringe
• Dilute to 5 ml
• Give 0.1 ml per kg

Does not readily cross the blood brain barrier as it is a quaternary amine (unlike the tertiary amine physotigmine) so not as useful in the treatment of the central side effects of anticholinergics (eg confusion). refl.in/fc

Inhibits acetylcholinesterase & impairs the hydrolysis of acetylcholine. The increased accumulation of acetylcholine competitively inhibits the non-depolarising muscle relaxants.
