Current ARC Guidelines recommend that air should be used initially, with supplemental oxygen reserved for infants whose condition does not improve during the first minutes of life. refl.in/8v
In the event of unexpected subglottic obstruction (eg tracheal web):
Call the ENT surgeons in.
Ensure mask ventilation is adequate, if not, pass size 5 bougie & railroad size 2.5 ETT.
Backup: 16g Needle Tracheotomy (Cricothyroidotomy may be difficult - see comment below)

Cricothyroidotomy practically impossible in neonate, would suggest needle tracheotomy.

PEEP 5cm H2O,
& for preterm infants;
PIP 25cm H2O,
PEEP 5cm H2O.

Meconium stained liquour? Delay tactile stimulation to avoid gasping in the infant with an oropharynx full of meconium.

If meconium stained liquour is present and the infant is non-vigorous, perform laryngoscopy and perform subglottic suction, or suction pharynx to 5cm with 12/14g catheter if unable to perform laryngoscopy. refl.in/8t.

In the vigorous infant (good muscle tone, active breathing efforts, HR >100BPM), provide standard care and do not suction. refl.in/8u
