If cuff leak when using double lumen ETT, disconnect tube, deflate both cuffs, advance tube, inflate both cuffs, and reconnect tube.

Managing hypoxia in one lung ventilation
- Scan monitor, other vitals stable?
- Confirm 100% O2 delivery.
- Notify surgeon.
- Assess tube depth & placement whilst ausculating lungs, ruling out pulmonay oedema/bronchospasm.
- Adequate paralysis?
- PEEP to dependent lung.
- CPAP or intermittent ventilation to non-dependent lung.
- If no improvement, ask surgeon to clamp pulmonary artery of non-dependent lung to redirect blood flow to ventilated lung.
- Consider ending case

Use a left sided double lumen tube unless surgery/pathology is at the proximal left main bronchus. Right sided tubes are more difficult to place as they are likely to obstruct right upper lobe.
