Keep in mind that reinforced ETTs have no murphy's eye, and have been known to obstruct when lumen is abutting mucosa. In the event of difficult ventilation consider withdrawing the ETT a little to see if it helps.
Maintain a pCO2 28-30mmHg until dura is reached, then maintain at 32mmHg for duration of case refl.in/5b
