Ask yourself, is this a patient with a hypertrophied RV that may have an RV dependent coronary circulation ( presence of myocardial sinusoids ) or is this a patient with hypoplastic RV?
The patient with RV dependent coronary circulation requires maintenance of a high intraventricular pressure through the maintenance of adequate preload to avoid myocardial ischaemia.
