In severe acidemia consider reducing the administered dose of thiopentone.

Thiopentone is a weak acid with a pKa of ~7.6 and in acidemia, the unionised fraction (fraction that crosses the blood brain barrier) increases.

Acidemia also increases the myocardial depressant effects of thiopentone.

The pKa of thiopentone is approx 7.6.

The unionised fraction is fraction that crosses the blood brain barrier, and in acidemic patients the unionised fraction is higher.
