If exchanging a tracheostomy tube for an oral or nasal ETT, the tube will tend to exit via the tracheotomy. Ask the surgeon to put their finger in the hole to prevent this from happening. MC #RCH

If resuscitating a patient with a tracheostomy, consider the possibility that the tracheostomy may be fenestrated in which case it will be difficult to deliver positive pressure ventilation without it leaking out the mouth.

Replace the inner cannula with an unfenestrated inner cannula (kits usually come with a fenestrated and unfenestrated inner cannula)
