A trifascicular block refers to a conduction block consisting of a right bundle-branch-block plus either :
1. left anterior hemiblock OR
2. left posterior hemiblock plus a 1st degree AV nodal block (not technically a fascicle, but then, "trifascicular" is a misnomer).
The patient is at risk of complete heart-block perioperatively and should be referred to a cardiologist where possible.

ECG findings.
1. RBBB.
2. Left anterior hemiblock (left axis deviation - lead I predominantly positive, lead II and AVF predominantly negative) or left posterior hemiblock (right axis deviation - lead I negative and AVF positive)

Left posterior hemiblock is less frequent than left anterior hemiblock because the posterior division of the left bundle is thicker & shorter and has a much better blood supply than its counterpart. refl.in/1i

If emergency surgery is required, consider how you will manage complete heart block if it develops perioperatively. Ensure transcutaneous external-pacing or transvenous pacing equipment is available in theater. Discuss with cardiologist if time permits for insertion of temporary pacing wire. refl.in/1i #safety
