Cardiopulmonary bypass places these patients at risk of cerebral hypoperfusion due to increased runoff of output into the pulmonary arteries.

In these patients as many MAPCAs are ligated and mobilised as possible (monitoring for arterial desaturation as an endpoint) prior to commencement of bypass.
