Neonates
Loading dose of 15 mg/kg then IV:
Preterm: 10 mg/kg/dose 24 hrly
Term: Week 1 of life: 10 mg/kg/dose 12 hrly
Week 2-4 of life: 10 mg/kg/dose 8 hrly
Severe infections: 15 mg/kg/dose
IT,Intraventricular: 5-10 mg once daily
Infants and Children
IV: 10-15 mg/kg/dose 6 hrly
IT,Intraventricular: 5-10 mg once daily (up to 20 mg/day in children)

Avoid rapid IV administration. Do not exceed a rate of 10mg/min. Vancomycin can cause significant reduction in systemic vascular resistance and result in profound hypotension. refl.in/dn
