--- Using the 4:2:1 rule ---
A useful way of approximating the required stat intravenous dextrose dose to treat hypoglycemia in the absence of a reference is to use the familiar 4:2:1 rule for intravenous maintenance fluid.
Calculate the hourly maintenance dose of 5% Dextrose and give this as a stat dose (although reducing the volume by administering 50% or 10% dextrose is usually more appropriate)
For example, the maintenance for a 4kg child is 16ml/hr 5% Dextrose. Therefore the required stat dose of 50% dextrose is approximately 1.6ml.
Rounding up is probably a good idea.
