If there is tumour around the duodenum, the patient is at risk of gastric outlet obstruction and residual gastric volumes. Discuss the likelihood with endoscopist and if so, consider a rapid sequence-induction

The endoscope is larger in diameter and stiffer at the end than a normal gastroscope and will be more stimulating at insertion. Additional analgesia will help reduce the response to insertion.
